Return Patient Guide
Patients with an appointment
1. Automated Check-In
Insert your patient ID card into the automated check-in machine.
You will receive two Registration Slips:
Exam Room Copy
To be used while waiting and for examinations.
Patient Copy
Please retain your copy until billing has been settled.
2. Registration
- Patients without preliminary examinations
- Please complete registration at least 15 minutes before your scheduled appointment time.
Example: Registration for a 10:30-11:00 appointment should be completed by 10:15. Registration for an 8:40-9:00 appointment should be completed by 8:25.- New patients may require more time to register and are advised to arrive slightly earlier.
- Patients with preliminary examinations
- Please complete registration before any scheduled preliminary examinations.
- Preliminary examinations (blood and urine tests, x-rays, etc.) should be completed an hour or more before your appointment time to allow for test results to be issued.
- Blood and specimen collection, x-rays, conducted from 8:00 a.m.
3. Health Insurance Confirmation
If your Registration Slip states thats health insurance confirmation is required, please present your health insurance card to Return Patient Reception on the 1st or 3rd floor of Building 2.

4. Preliminary Examinations
- Patients without preliminary examinations
- Please proceed to your consultation room.
- X-ray, Mammography, DEXA, GI Series, CT, MRI
- Please proceed to X-Ray Reception for instruction.
- Other examinations
- Please proceed to your examination room.
You will receive a check mark on your Registration Slip once testing is completed.your reception slip.
5. While You Wait

- Place the Registration Slip in the tray at the front of the room.
- Please wait near the room for your name to be called.

- Your number will be displayed on the screen.
- Before you are called, your number will be displayed on the screen in the waiting area.
- 1The numbers of patients currently being seen are displayed on the screen.
- 2The numbers of patients about to be called are also displayed on the screen.

- Your name will be called.
When your full name is called, please enter the room.
- To prevent patient misidentification, you will be asked to give your full name and birthdate upon entering the room.
6. Today's Treatment Schedule
After your examination you will receive a treatment schedule, which will detail any further tests and treatments for the day, as well as billing information. Please follow these steps.
- 1 You will receive a check mark next to each completed step.
- 2 Examination location and instructions appear on the sheet.
- 3 If there are check marks in the Notes section at the bottom of the sheet, please follow the instructions found to the right of each mark.
7. Billing & Payment
After completing all examinations and treatments, please proceed to the cashier.
- 1Please take your colored file, which includes your treatment schedule for the day, and your patient ID.
- 2You will receive a billing reception slip.
- Available only at Reception Window 20 on the 1st floor.
- 3Once your bill has been calculated, your number will be displayed on the screen.

- 4Please pay your bill at an automated payment machine.
- Payment can be made by cash or credit card.
- Receipts cannot be reiussed. Please keep your receipt in a safe place.
8. Your Next Appointment
Your next scheduled appointment time is printed on the Patient Information Guide attached to your receipt.
- Appointments are listed with priority given to departments seen during your current visit.
9. Filling Your Prescription
If you receive a prescription, a prescription voucher will be attached to the right side of your receipt. Please tear off this voucher to the Prescription Counter (Reception Window 25).

- If your previous hospital visit occurred within the last six months, please proceed to the Initial Visit counter.
Walk-in consulations are conducted at the discretion of our physicians. You may be asked to make an appointment for another day.3rd Floor Reception - Dentistry & Oral Surgery, Anesthesiology, Neuropsychiatry
1st Floor Reception - all other departments - If your previous hospital visit occurred more than six months ago, please proceed to the 1st floor Initial Visit counter.
- If your previous hospital visit occurred more than six months ago, or if have new symptoms, you are encouraged to first consult a nearby health care center, then visit our hospital with an appointment.
- Visits made after having been referred back to another hospital, or without a referral letter, will incur an Extra Consultation Fee of 2,750 yen (tax included).
