- This step can be skipped by those who have already had an appointment made through another health care center.

When calling, be prepared to give the following information:
- Whether you have a referral letter
- The name of the health care center that issued the referral letter
- Physician or department you wish to consult
- Desired appointment date and time
Sending Your Referral Letter in Advance
We recommend that referral letters and any other medical information be sent in advance.
The Benefits of Sending Your Referral Letter in Advance
- Minimizes check-in time and allows you to proceed directly to your examination room.
- Your physician is able to review your records in advance, and may be able to arrange examinations that would usually require a second visit.

Mailing Your Referral Letter
(For privacy protection, please send the letter by delivery-certified mail.)
Referrals letters must be received within two days of your appointment.

Receiving Your Appointment Notice (Pick Up Only)
You will receive an appointment notice. Please bring it with you on the day of your appointment.
- If your appointment was made through another health care center, they will issue the appointment notice.

- Arriving at the Hospital
Arrival times are staggered to reduce crowding.
Please arrive at your appointed time.
If you plan to bring your referral letter and related documents, they will need to be registered. Please inquire at New Patient Reception.
