Appointments

Please visit Admissions & Discharge once your admission has been scheduled. Staff will confirm any room requests and explain the admission process.
You may also be asked to give any personal information that will help ensure smooth provision of care and testing during your stay.

If you are unable to visit the hospital

If you are unable to visit the hospital, a proxy may make the appointment in your place. When it has been scheduled, Admissions & Discharge staff will contact you regarding your admission date.

Admission Date Confirmation

The Admissions & Discharge center will call you on the business day preceding your admission day (e.g. Monday admissions will be contacted the previous Friday). Please make all necessary preparations so that you can be admitted on your scheduled day. Phone calls are placed between 9:00a.m and 4:00p.m. If you are unavailable to receive the call, it is your responsibility to make a return call to the hospital.

Cancellation

If you wish to reschedule or cancel your scheduled admission date (due to unavoidable circumstances only), please contact Admissions & Discharge (Reception Window 15) immediately.

Admission Refusal

To prevent the spread of infection, you may be refused admission if you have, or are suspected of having rubella, mumps, or chickenpox.

Hospital Policies
  • The hospital does not handle deposits, and cannot hold cash, passbooks, or other valuables. To avoid theft, please do not bring large amounts of cash or valuables to the hospital.
  • Please refrain from using courier services to send personal belongings to the hospital.

What to bring

Admission Procedure

Health insurance card and any other medical documents.
  • –Please be sure to bring your health insurance card. If you do not present your card, all expenses may be charged to you.
  • If your health insurance card has been renewed after your admission to the hospital, please present your new card at Admissions & Discharge (Reception Window 15) immediately.
  • Health insurance cards and medical treatment vouchers are photocopied when necessary, and may not be immediately returned.

Medical Documents

Admission Consent & Application Forms
Forms are to be completed and submitted upon admission to the hospital.
Hospital Card
Your hospital card is required for admission procedures and for payment at time of discharge.
Nursing Sheet & Database
Before admission, you will be asked for details of your health and lifestyle status as well as your family status that will be referenced during and after your stay. Forms will be given to you in the clinical examination room.

What to Bring

Toiletries

Towel, bath towel, toothbrush, shaving kit, liquid soap, shampoo, etc.

  • Please check your room as some amenities may be provided.

Utensils
Cutlery of choice, plus two durable cups (for meals and brushing teeth)
Daily Necessities

Tissues, indoor footwear (Non-slip soles recommended. Avoid slippers and sandals.), pen and paper

Clothing
Dressing gown or pajamas, underwear, outerwear (gown, cardigan, etc.)
